I. Basic Services of Funeral Director and Staff

Our basic service charge includes, but is not limited to, staff response to initial request for service, consultation with family or responsible party, preparation and filing of necessary certificates and permits, placement of obituary notices, planning of funeral arrangements, and coordination of services with cemetery, crematory, vault companies and others as required. A Licensed Funeral Director and Embalmer is available 24 hours a day, 365 days a year. It also includes certain overhead items associated with the operation of the funeral home, such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

II. Embalming

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

III. Other Preparation of Body

- Sanitary care, washing and disinfection when no embalming.
- Special cosmetics when no embalming.
- Restorative work when necessary(per hour rate).
- Extra charge for preparation of autopsied body.
- Hair Dressing.

IV. Additional Services and Facilities

A. Use of Facilities and Staff for Visitation
Our services will include visitation setup, use of visitation room or at off-site facilities; placement of casketed deceased, display of floral arrangements, parking lot assistance, and supervision and attendance during visitation.

B. Use of Facilities and Staff for Ceremony
Our services include coordinating the funeral arrangements, supervision of funeral, staff to attend to the funeral ceremony at the funeral home or off-site funeral services, supervision of committal or other disposition service, setup and use of facilities for funeral ceremony.

C. Memorial Service
Our services include coordinating the memorial service, supervision of the memorial service, staff to attend memorial service, and setup and use of facilities for memorial service.

D. Graveside Service
Our services include coordinating the funeral arrangements at graveside, supervision of the graveside service, and equipment and staff necessary to conduct the graveside service.

VI. Caskets and Alternative Containers

A casket list will be provided at the funeral home. Alternative containers are a container that "encases the body". We use cardboard containers.

NOTE: Family made containers are permissible provided they are delivered to the funeral home and a waiver of liability is signed.

VIII. Forwarding Remains to Another Funeral Home

This charge includes the transfer of the deceased, basic services of staff, embalming, necessary authorization and local transportation to airport. This charge does not include shipping container, visitation, rites, or ceremonies prior to forwarding of the deceased.

IX. Receiving Remains From Another Funeral Home

This charge includes temporary shelter of deceased, basic service of staff and transportation of deceased to cemetery or crematory. This charge does not include visitation, rites or ceremonies.

X. Direct Cremations

This charge includes transfer of deceased, local transportation to crematory,sanitary care as required by the State of Massachusetts, crematory charges,Medical Examiner fee required by the Commonwealth of Massachusetts, and basic services of staff. This charge does not include cremation container required by crematory or the use of facilities and staff for any visitation or ceremony, public or private, prior to cremation.

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composite materials (with or without an outside covering). The containers we provide are cardboard containers.

XI. Immediate Burial

The charge for immediate burial includes transfer of deceased, local transportation to the cemetery, the necessary authorizations, sanitary care, and basic services of staff. The charge does not include the use of facilities and staff for any visitation or ceremony, public or private,prior to burial.

XII. Anatomical Gift to University of Mass. At Lowell

The charge for anatomical gift to University of Massachusetts, LOWELL Campus, includes transfer of deceased to funeral home, transfer of deceased to the college, transfer of deceased to place of final disposition (local), embalming according to the Medical Schools' specifications, necessary authorizations, basic services of staff. The charge does not include the use of facilities and staff for Crematory or Cemetery opening fee.

**NOTE** At this time the University will pay $1,100.00 towards the expenses of final disposition of the anatomical gifts.
